Under federal law (Paperwork Burden Reduction Act), we are not required to mail ACA Form 1095-C to all employees. However, you have the right to request a copy.
What is Form 1095-C?
This form provides information about the health coverage offered to you by Tarrant County. It may be helpful when preparing your tax return, but it is not required to file your taxes.
How to Request Your 1095-C: Fill in your details on this form
Deadline for Requests:
We will provide your form within 30 days of your request or by January 31, 2026, whichever is later.
Who Can Request:
- Current employees
- Former employees who were offered coverage during the prior year
- Retirees, under 65, who were offered coverage during the prior year
