General Instructions

This form may be used by health care providers, hospitals, pharmacies, schools to report confidential information of suspected cases and confirmed cases of notifiable conditions in Texas to the Tarrant County Public Health Department. A list of notifiable conditions can be found at https://www.dshs.texas.gov/notifiable-conditions. In addition to any outbreak, exotic disease, or unusual group expression of disease that may be of public health concern should be reported by the most expeditious available, meaning a phone call to 817-321-5350.
