Continuum of Care Program
Continuum of Care Program
The Continuum of Care (CoC) Program is designed to develop supportive housing and services that will allow homeless persons to live as independently as possible. Eligible applicants are States, units of local government, other governmental entities such as Public Housing Authorities (PHAs) and private nonprofits.
Community Development Department (CDD) administers funds for multiple non-profit agencies that provide housing placements, supportive services, and property‑related inquiries. CDD does not place individuals into housing, manage rental properties, or operate sober‑living residences.
Landlords or sober‑living operators seeking partnerships may contact the local CoC Landlord Engagement Team at Partnership Home. The Team manages property partnerships on behalf of the Continuum of Care.
