The Environmental Health division provides health-related services involving commercial food establishments, public pool and storm water quality, septic system standards, vector control and other quality of life issues in Tarrant County. Our web pages feature buttons/links to forms that are required for specific services. You can download and print the forms by clicking on their highlighted names.
Many of these forms are interactive and can be completed on your computer, or printed and filled in by hand. Once the forms are completed, you can submit the permit applications in person or by mail to Environmental Health at the address in the CONTACT section above, or to the email link below.
