Online Report Form

WHAT YOU NEED TO KNOW BEFORE YOU SUBMIT YOUR ONLINE REPORT FORM

  • Your community supervision counselor must have already granted you online reporting privileges in order for you to be able to use this form.
  • You will need the PASSWORD provided to you by your counselor in order to access the online report form.
  • You must report every month between the 1st and 25th day of the month.
  • Any report form submitted after the 25th day of the month will NOT be accepted.
  • Failure to comply with the court-ordered terms and conditions of your community supervision, including reporting as directed above, will result in your online reporting being suspended and you will have to report in person.
